Frequent Disconnects

Hello,
I am facing problems of disconnection frequently nowadays. It started about a month ago, the Internet LED lights go off for like 3-4 seconds and they again come back and
internet works fine again. This is not a big problem while casual browsing, but when I am playing MMO's and uploading or downloading files, it is really saddening.
I disconnect from the internet every now and then, and I cannot find the reason behind it.
Here is the system log.
P.S - Please ignore the date differences in the log, I made changes to the date..
